Enhancing surgical education for resource-limited settings: open appendectomy cognitive task analysis

Open appendectomy is one of the most common procedures performed globally, especially in low- and middle-income countries (LMICs). In many LMICs, surgical education limited by personnel shortages. Developing effective teaching methods for resource-limited environments may enhance availability a trained workforce. Cognitive task analysis (CTA) an method to obtain expert input define operative steps cognitive processes complex procedures. The aim this study was perform first CTA open effort develop module practitioners LMICs. Twelve general surgeons completed semi-structured interviews which were recorded via Zoom. Each had previously > 100 appendectomies. interviewee described entire procedure then answered specific questions inform decision making, anatomic landmarks, error recognition/avoidance. responses collated two independent reviewers into model underwent review through multi-disciplinary team. Final presented back experts feedback modifications tested on simulator. Completeness each expert’s interview determined metrics: time percentage compiled that surgeon described. identified five main steps: preoperative patient preparation, safe entry abdomen, identify appendix, appendectomy, abdominal closure. step additional sub-steps total 24 procedural steps. Available equipment/supplies critical point descriptions provided variability. median completeness score unstructured portion 76% (IQR 68–80). Using CTA, we defined essential steps, points, areas potential with performing appendectomy. These results provide framework can guide intraoperative instruction development be implemented LMIC education.
